LDAP connection UI looks broken, constantly prompts for password

The LDAP connection UI does not properly show when a password has been set for simple authentication.   After saving login credentials, it immediately shows the password as empty, with an error stating "this field is required".    If you enter a password, it again shows an empty field with "this field is required".  This makes the connection screen look completely broken.

Expected: If a password has been saved, the UI should show that it has been saved.
